How to Apply and Operate

the insurance application screen

 

Please follow these instructions to apply for insurance.

 

*Note

To return to the previous screen, please click the back button on the bottom left of the screen, not the browser's refresh/back button.

1. Apply from connect job

Please fill required information on connect job page and click “apply”.

 

２. Redirect to Tokio Marine's website

You will automatically be redirected to Tokio Marine’s website.

Click 「次へ進む(next)”」.

 

 

３. Go to the website

Click 「お手続きはこちらから(apply from here)」.

If you are using a smartphone, the button will be on the bottom of the screen.

 

 

４. Go to the website

Click 「次へ進む(next)」.

5. Fill in the form

①Fill 「名前（Name）」「生年月日（date of birth)」「性別(sex)」「ご職業(occupation)」

　 For 「職業(occupation)」, choose the one that is closest to your occupation.

②Please read 「意向確認(confirmation of your intentions)」.

③Click 「次へ進む(next)」.

 

6. Select your plan

① Select the package plan you want.

② Click 「選択する(choose)」.

 

 

7. Select the coverage 

①Coverage marked 「選択中（Selecting）」 is coverage that you already plan to subscribe to. Compensation marked as 「未加入（Not subscribed）」 is optional.

If you choose the "Premium" plan, coverage for the blow items is already selected.

• ケガの保険（ほけん）(Injury coverage)
• 病気やケガのほけん(Healthcare coverage)
• ガンの保険(Cancer coverage)
• 個人（こじん）賠償(Personal liability)
• 借家人賠責(Tenant’s liability)
• 生活動産(Personal property for living in housing)
• 救援車費用(Rescue expenses)

 

8. Select the coverage 

①   Consider whether you would like to sign up for additional coverage that is 「未加入（Not subscribed）」

②  If you would like to sign up for additional subscriptions should press the 「加入を検討する（Consider joining）」 button.

The procedure for signing up for additional 「所得（しょとく）補償（Income Coverage）」

① Select 「所得補償基本給別（By income compensation base salary）」　ｔo find out which one you should choose, please check 「所得補償基本給別につきましては、こちらをご確認ください。」 to find out which one you should choose.

②   Click 「次へ進む(next)”」.

 

① Click 「選択する(choose)」.

①  The display changes from 「未選択（Not selected）」 to 「選択中（Selecting）」.

②  Select 「確定する（Confirm）」

9. Confirm the coverage  

①Click 「補償を確定し次へ進む（Confirm compensation and proceed to the next step）」

 

 

10. Income coverage_answer for health status(1)

①   Choose an answer 「なし（NO）」 or 「あり（YES）」 for the question “Are you currently hospitalized for an illness or injury, or have you been advised to undergo hospitalization surgery?”

②   Click 「選択する(choose)」.

 

 

11. Income coverage_answer for health status(2)

①  Choose an answer 「なし（NO）」 or 「あり（YES）」 for the question　”Within the past year, have you been continuously hospitalized for more than 10 days due to illness?”

②   Click 「選択する(choose)」.

 

12. Income coverage_answer for health status(3)

①  Choose an answer 「なし（NO）」 or 「あり（YES）」 for the question “Have you ever been diagnosed by a physician with cancer, epithelial cancer, or mental illness (including drug or alcohol dependence)? Has your doctor ever recommended that you undergo testing or treatment for them?”

②   Click 「選択する(choose)」.

 

13. Cancer coverage_answer for health status(1)

 ① Click 「告知の大切さに関するご案内（Guidance on the importance of notification）」

②    Check the content that comes up.

③   Click the [x] button.

 

14. Cancer coverage_answer for health status(2)

①Choose an answer 「なし（NO）」 or 「あり（YES）」 for the question “Have you ever been diagnosed by a physician with cancer or epithelial cancer?”

② Click “次へ進む(next)”.

 

 

15. Cancer coverage_answer for health status(3)

①  Choose an answer 「なし（NO）」 or 「あり（YES）」 for the 2 questions.

Have you had any of the following within the past two years?

1. Have received any of the following tests in a medical checkup or physical examination, and as a result, have had an abnormality indicated in an organ or test result (requiring treatment, requiring close examination, or requiring reexamination within one year).
2. Have been instructed by a physician to undergo ongoing medical examination (including medication and treatment) and tests due to the following illnesses, initial observations, or symptoms.

・Polyps, tumors, etc.

Tumors, nodules, masses, GIST, carcinoid, dysplasia,

leukoplakia, multiple polyps (polyposis), abnormal pathology or cytology

・Diseases of the digestive system

Cirrhosis, chronic hepatitis, liver dysfunction (with hospitalization or treatment), chronic alcoholic liver dysfunction, NASH (nonalcoholic steatohepatitis, alcoholic hepatitis, portal hypertension, esophageal varices)

・Respiratory disease

COPD, emphysema, chronic bronchitis, pulmonary fibrosis, pneumoconiosis, silicosis, interstitial pneumonia

・Kidney Diseases

Chronic renal dysfunction, chronic renal failure, chronic nephritis, uremia

・Other diseases

Hepatitis B virus carriers, hepatitis C virus carriers, anemia (except iron deficiency anemia)

・Other symptoms

Lumps, bleeding (irregular bleeding, hemoptysis, hematemesis, hematuria, internal hematuria), jaundice

②Click “次へ進む(next)”.

 

16. Hospitalization coverage_answer for health status(1)

①Choose an answer 「なし（NO）」 or 「あり（YES）」 for the question “Are you currently hospitalized or recommended for hospitalization or surgery for an illness or injury?”

②Click “次へ進む(next)”.

 

 

17. Hospitalization coverage_answer for health status(2)

①Choose an answer 「なし（NO）」 or 「あり（YES）」 for the question  “Have you been hospitalized for more than 10 days continuously due to illness within the past year?”

②Click “次へ進む(next)”.

 

18. Confirm the information

①Check your information. If there are any errors, please correct them.

②Click “次へ進む(next)”.

 

19. Verify all the information

①Verify all the information.

②If the compensation plan you have chosen is in line with your opinions and feelings, checｋ☑ the 「確認しました（I have confirmed）」 box.

③Click　「確認しました（Confirm）」.

 

④Click [OK」for the question “Once the details have been finalized, no changes can be made.”, if you agree.

20.  Verify all the information(2)

 

① Click「重要事項説明書を表示する（show “important information sheet”)」.

②Please check 「重要事項説明書(important information sheet)」「加入内容確認事項(Confirmation of Subscription Details)」「告知の大切さに関するご案内(Information on the Importance of Notification)」.

③Click [×]

④ Read the rest of the description, if necessary.

⑤ Click 「加入する(Subscribe to the insurance)」.

21. Go to the next step to register your payment method

① You will receive an email to your own email address.
② When you click 「次へ進む(Next)」, you will be redirected to a page where you can register your payment method.
